ADMISSION
Children are eligible for enrollment in the Wee Warriors Preschool program if they will be three (3) years of age by August 31st and are completely toilet-trained.
Children are eligible for enrollment in the Wee Warriors Pre-Kindergarten program if they will be four (4) years of age by August 31st and are completely toilet-trained.
A completed APPLICATION FOR ADMISSION and registration fee is required to guarantee your child’s enrollment. A non-refundable Registration Fee of $25.00 per child is due each year upon acceptance into a classroom. Checks should be made out to CCC (Please write, “Wee Warriors Preschool” in the memo).
The following forms must also be completed and returned prior to your child’s first day of preschool (mouse over forms for link):
- KS Dept. of Health and Environment (KDHE) Medical Record, Immunization History and Child health Assessment
- KDHE Authorization for Emergency Medical Care
- Emergency Contact Information
- Pick-Up Authorization Form
- KDHE Parental Permission Form for Off-Premise Trips (if applicable)
- Handbook Signature Page (last page of handbook)
- Authorization of Consent for Medical/Surgical Treatment
